RF microneedling and facial radiofrequency are among the most requested firming treatments — and among the worst explained. A “non-surgical facelift” gets promised, an impossible before-and-after gets shown, and the patient arrives expecting something no radiofrequency device can deliver.

Here is what actually happens in the skin, what to expect, and on what timeline.

What radiofrequency does inside the skin

Radiofrequency emits high-frequency electromagnetic waves that pass through the epidermis without damaging it and generate heat in the dermis at a controlled depth. That heat produces two distinct effects on two different timelines:

  1. Immediate contraction of existing collagen. Above 40–42 °C, collagen fibres shorten. This is the effect you see walking out of the room, and it lasts a few days.
  2. Neocollagenesis. Controlled heat is read by the tissue as a micro-injury and activates fibroblasts, which build new collagen and elastin over the following weeks. This is the real, lasting effect.

Which is why radiofrequency is never a one-session treatment. The first session gives a pretty, short-lived result; the result that holds is built between weeks four and twelve.

Radiofrequency, RF lifting, RF microneedling — the same thing?

The names get used interchangeably and it doesn’t help. The real difference is how the heat reaches the dermis.

Modality How the heat arrives Depth Main indication
Non-invasive radiofrequency Through intact skin, via a handpiece Superficial and mid dermis Light firming, glow, maintenance
Fractional RF microneedling Microneedles deposit energy below the skin Mid and deep dermis, adjustable Moderate laxity, pores, acne scarring
RF + CO₂ (Fraxis Duo) Laser ablation and RF energy in one protocol Epidermis and deep dermis Texture, post-acne and firmness in one session

Spanish searches say “radiofrecuencia facial”, Russian says “РФ лифтинг”, English says “RF microneedling”. One family of technologies, three commercial names. Details on the RF microneedling page.

What realistically improves

What radiofrequency does not do: it does not reposition sagging tissue and does not replace a surgical facelift. Where there is structural descent, the right tool is focused ultrasound — SMAS lifting with Ultraformer III, which reaches the muscular layer. The best plan often combines both: HIFU to lift, radiofrequency for skin quality.

How many sessions, and how often

Goal Sessions Interval Maintenance
Glow and light firming 4–6 Every 2 weeks 1 session every 2 months
Moderate laxity (microneedling) 3–4 Every 4 weeks 1–2 per year
Acne scarring 4 Every 4–6 weeks As the skin responds
Neck and décolleté 4–6 Every 2–3 weeks Quarterly

Before and after the session

The non-invasive session runs 30–45 minutes, feels like intense but comfortable heat, and leaves no marks — you can put make-up on and go. The microneedling version needs topical anaesthetic, leaves the skin red for 24–72 hours and demands strict sun protection for two weeks.

In both cases, avoid sauna, hard exercise and direct sun for 48 hours. On the Costa Blanca that last point matters more than elsewhere: scheduling fractional RF between October and April gives better results and a lower risk of hyperpigmentation.

Frequently asked questions

When will I see results from RF treatment?

There is an immediate smoothing effect that lasts a few days. The real result appears between weeks four and twelve, once new collagen has formed, and keeps improving until month six.

How long does it last?

Twelve to eighteen months with a full protocol, provided it is maintained with occasional sessions. Skin carries on ageing: radiofrequency slows the process, it doesn’t stop it.

Does it hurt?

Non-invasive radiofrequency doesn’t hurt — you feel heat. The microneedling version is uncomfortable, which is why topical anaesthetic goes on 30 minutes beforehand.

Is it safe on darker or tanned skin?

Radiofrequency is among the safest technologies for higher Fitzpatrick types because it doesn’t target melanin, unlike laser. Even so, freshly tanned skin is not treated.

Can it be combined with fillers or HIFU?

Yes. Radiofrequency usually goes first and fillers afterwards, or two weeks apart. Combining with HIFU is planned within a full face treatment.

What if I have a pacemaker or an IUD?

A pacemaker or other implanted electronic device rules radiofrequency out. A copper IUD rules out abdominal body treatment, not facial treatment. Always declare it at the assessment.
